The globe is ageing and with this phenomenon challenges arise. Alternative formal care systems are replacing informal family care. To be able to provide optimal care it is mandatory to put the elder's demands as a priority. The elderly population is heterogeneous and so are its needs. Most studies have found a gap between what health professionals believe are essential needs of elderly and the actual demands of the elderly. Therefore it is similarly essential to implement an adequate assessment tool to investigate the unique requirements of every elder before joining any model of care. Hence, the service provided is tailored to the preference and requirements of every elder
