Beschreibung

Hypertension, as an example of chronic diseases, poses a general health problem worldwide. The problems in blood pressure control are related to failure in adherence to treatment, prescribing proper medications, and lack of discrete follow-up.Clinical pharmacists are qualified to participate in solving these problems through the new discipline of Pharmaceutical Care.A consensus should be reached by health care policy makers, about how this new practice will fit in the whole process of patient's care.A model of this practice is investigated in Saudi Arabia outpatients'hospital clinic, which targets the effect of adherence, knowledge, and drug problems resolving,on blood pressure control. The results illustrated how a team work of pharmacists, doctors and nurses would result in a real improvement in quality of care. This study targets policy makers, researchers, pharmacists and other health professionals.Pharmaceutical care should be a genuine addition to the complete picture of the Health Care System.
