An incompletely filled Biochemistry request form is a common problem faced by the laboratory. Clinical laboratory testing is an invisible side of medical care. On the basis of laboratory data clinicians take decision about diagnosis, developing a course of therapy, monitoring response to therapy. Communication between laboratories and use of laboratorys service by clinician for patient care is mainly through laboratory requisition form. Incomplete filling of requisition form can significantly affect quality of laboratory report and patient care. In the benefits of patients, any direct or indirect undesirable consequence related to a laboratory test must be considered, irrespective of which step is involved and whether the error is caused by a laboratory professional (e.g., calibration or testing error) or by a non-laboratory operator (e.g., inappropriate test request, error in patient identification and/or blood collection). This book provides information about importance of filling of biochemistry requisition form and its impact in quality of care of the patient.

Dr. Shruti Singh is an Associate Professor in the Department of Biochemistry and Molecular Biology.Dr. Singh received her Doctor of Medicine (MD), a three-year postgraduate doctoral course in Biochemistry from B. P. Koirala Institute of Health Sciences (BPKIHS), Dharan, Nepal in 2013.
